Birthday Requests and Mentions
Requests for mentions can only be accepted UP TO 7 DAYS BEFORE the performance date. After this time, late applications should be submitted to a member of staff at the venue immediately before the performance.

- Venue
- Date of performance
- Time of performance
- Your name
- Name of person to be mentioned
- Any other information i.e. their age, birthday etc...
